A stem cell clinical trial first launched in 2004 to evaluate the safety of Neural Stem Cell Transplantation in patients with chronic Spinal Cord Injuries (SCI) is about to be conducted by scientists, and the results of these tests could offer renewed hope among victims.

These researchers are from three collaborating organisations, the UC San Diego School of Medicine, the Sanford Stem Cell Clinical Center at UC San Diego Health and Neuralstem, a Maryland-based biotechnology company.

This trial has been expanded to include four more participants with chronic SCI. These trials will involve a very intensive follow up schedule, and only a line of stem cells approved by the Food and Drug Administration of the United States will be used. The good news for the participants is that these stem cells have been tested for safety on patients with Amyotrophic Lateral Sclerosis (ALS).

The researchers will only be considering participants within a 500 mile radius of San Diego. Should any of the participants not live within the 500 mile radius, it is recommended that they take up residence within this radius for the period that these study will be conducted.

Objective of the Study

The objective of this study is to determine the safety and the toxicity of the treatment, which involves surgical procedures with six stem cell injections and a follow-up period of 60 months.

The researchers want to develop an effective treatment for chronic SCIs and also to determine whether injecting Neural Stem Cells into patient’s spines pose any danger to them.

Surviving a grisly car accident is always a welcomed luck to anyone involved. However, suffering severe injury to the cervical spine that leaves you paralysed from the neck down is devastating. One such case is Kristopher Boesen (also known as Kris) from Bakersfield.

In March, Kris lost control of his car and crashed into a tree and a lamppost. His doctors were not very hopeful about his prognosis. They informed Kris’ parents that he might be permanently paralysed from the neck down.

Stem Cell Research

Researchers are studying a new Stem Cell Treatment procedure that may have the potential to improve the lives of patients suffering from a traumatic injury to the spinal cord. The procedure which is at its clinical trials Phase 1/2a directly introduces AST-OPC1 cells into the patients’ spinal cord. AST-OPC1 cells are formed from embryonic stem cells which are converted in a lab.

Luckily, Kris qualified for this clinical trial that was undertaken by a team led by Dr Charles Liu from Keck Medical Center of USC. The procedure was completely experimental and did not guarantee any restoration of body function. “Typically, spinal cord injury patients undergo surgery that stabilises the spine but generally does very little to restore motor or sensory function,” Liu explained.

Improvements following Treatment

In April, Kris was injected with a dose of 10 million AST-OPC1 cells directly into his spinal cord. Two weeks later, he was showing signs of improvement. 90 days after the surgery, he was able to feed himself, write his name and operate his wheelchair. He also gained significant improvement in his motor function up to two spinal cord levels, said Dr Liu.

“With this study, we are testing a procedure that may improve neurological function, which could mean the difference between being permanently paralysed and being able to use one’s arms and hands. Restoring that level of function could significantly improve the daily lives of patients with severe spinal injuries.”, Dr Liu explained.

“All I’ve wanted from the beginning was a fighting chance,” said Boesen. “But if there’s a chance for me to walk again, then heck yeah! I want to do anything possible to do that.”

Australian bio-molecular scientist, Professor Alan Mackay-Sim who has had a lifelong interest in the regeneration and repair of the nervous system, has just been recognised as Australian of the Year. Professor Mackay-Sim earned this accolade following his pioneering work in using stem cells to treat a quadriplegic man. Mr Fidyka subsequently regained muscle control and feeling in the region beneath where he had been stabbed.

Most Spinal Cord Injuries are caused by trauma or damage to the spinal cord. The resultant impaired function translates to considerably reduced mobility and loss of sensation. A small but significant number of these injuries are caused by disease.

Broadly speaking Spinal Cord Injuries fall into two major categories: incomplete and complete.

An incomplete injury to the spine means some motor function is retained below the level of the injury. With appropriate and sustained rehabilitation one can make do with crutches and still retain a measure of independence.

The extensive nerve damage that results from a complete spinal injury effectively obstructs all signals from the brain to those parts of the body located below the injury. - Head injuries, particularly those with trauma to the face
- Pelvic fractures
- Penetrating injuries in the area of the spine
- Injuries from falling from heights

If any of these injuries occur together with any of the symptoms mentioned above (acute head, neck, or back pain; decline of feeling in the extremities; loss of control over part of the body; urinary or bowel problems; walking difficulty; pain or pressure bands in the chest area; difficulty breathing; head or spine lumps), then SCI may be implicated.
